Step 1: Go to the app > Dashboard > Settings

Step 2: Under the Settings > Survey Questions > Add New

Step 3: Add Question

Step 4: Select the Element For The Options 

Step 5: Add Values To The Options

Note:- 

• There has to be at least one question in the survey.
• The App requires at least one question enabled for the survey

Thus, mark one question as “Required” while you’re creating a survey.

Step 6: Save your settings by clicking on the “Save” button & view your survey on the Order Confirmation page.
